Pattern: Estou com + symptom is the default in Brazil — not Eu tenho dor (though Tenho febre is also heard).

Part 2 — Gramática: o verbo doer e estar com dor
Doer — to hurt / to ache
Doer works like gostar: the thing that hurts is the subject, and the person is an indirect object.

Both are natural. Estou com… is more common in conversation.
